Thank You Very Much for your decision to enroll your child (ren) in Super Kids Karate.   We have proudly joined with Public School Districts, DayCare and Private School Providers throughout the Northern California, Seattle, Washington Chicago, Illinois Regions to bring On-site Martial Arts Enrichment and Development Programs to your child.  

Our full-time Corporate Headquarters has been a proud member of the Sacramento community for over fifteen years.     Our programs have graduated over 150 Black Belt students – including Junior Black Belts as young as 7 years old and adult Black Belts of all ages.  In the last five years alone, we have trained over one thousand students at our Headquarters facility and an additional two thousand students at our On-site Public School, DayCare and Private School Programs.  In addition, we have developed a strong relationship with local school districts where we provide Motivational Assemblies and Black Belt Demonstrations in support of Red Ribbon Week, Drug and Violence Education and child safety. 

ENROLLMENT PROCESS

Step One:  Please understand that We Have Different Enrollment Forms across the Many Sites that we serve - so be careful to select the right Enrollment Form in Step Two!!!

Step Two:  Print and Complete the Enrollment Form below.  You will need Adobe software in order to open the file.   This software is available for free at www.adobe.com.   Your PC may already have this software loaded, so give it a click below!   You will need Adobe Reader to open and print these files.  If you do not have Adobe on your computer, download it for free by

Step Three:  Leave Enrollment Package and Form of Tuition Payment with Front Office Staff at Your Child's School

Step Four:  Super Kids Karate Staff will include your Child in the next Karate Class after the Enrollment Package is Completed 

Uniforms are $35 extra and will be delivered 2-3 weeks after your enrollment form is received at the Corporate Office.  

We use CHARGEBACKBUREAU.ORG to pre-check for fraudulent charges and chargeback oriented purchasers.  Customers having one or more credit card or EFT chargebacks for any reason will be required to sign additional paperwork or may be refused membership in our programs.   Members who initiate a chargeback against our company will be reported immediately and will have future credit card privileges at the other 57,000 member businesses denied.
